Click on the large <Questionnaire> button in the top centre of the screen and then on the
<Creating a new questionnaire> link.

The following screen will open:

Type in the name of your questionnaire. By default this name will be seen by the respondents, however you can override this later. Optionally you can add some further description. You can use this to store the name of the person initiating the survey, for example.

You can copy an existing questionnaire if you wish. This is very useful for periodically repeated surveys.
When done, click on the <Enter> button.

The questionnaire design screen opens:

That's it. You are now at the page where you can begin to compose your survey.
